I am a highly skilled Case Manager with extensive experience and a sound knowledge base.
As the managing director of Sarah Trinder Case Management.
I ensure our philosophy is one of collaboration, using a biopsychosocial approach to support the client and their families on their rehabilitative journey. During my career I have developed skills and knowledge as a Registered Nurse with over 25 years’ clinical experience in the areas of care of the elderly, acute surgery, general medicine, accident and emergency, neurology, palliative care, rheumatology and cardiac care.
Additional to my clinical experience I have further had a successful career managing a large nursing home, honing my skills in the holistic organisation of people with long term neurological conditions. Alongside which, I co-ordinated and worked in partnership with the multi-disciplinary teams.In 2017 I progressed into case management where I combined experience and passion for rehabilitation, to support clients living with brain injury and complex care needs.
With an excellent understanding of both NHS and private sector services and extensive experience managing early intervention and pre-litigation cases I have forged successful and strong relationships with clients, their families and associated agencies.
Holistically, I put our clients and family’s safety and care needs at the core of my work.

I am a qualified and registered Occupational Therapist who has experience of working with a wide range of clients, across a variety of health and social care settings. My skills, knowledge and passion for rehabilitation have helped me to establish strong therapeutic relationships with clients living with complex care needs.‍ I have worked with patients in adult acute, child and adolescent and forensic mental health services, acquired brain injury and disabled children’s services.‍ During which time I have  become competent with complex discharge planning and continuing care for those clients.‍Excellent communication and interpersonal skills have allowed me to develop effective and collaborative relationships with professionals in NHS, private and third sector agencies. I thrive in being part of a team and when faced with challenging clinical situations, I work objectively to reduce risk and obtain the best outcome for the client.‍ As a case manager I have worked with clients who live with enduring mental health conditions, traumatic brain injuries,
imputations, and cerebral palsy, part of which has included the recruitment and managing of care teams.‍I pride myself on being client-centred and goal-orientated; aiming to secure effective results for clients and their families.

Having worked in many different healthcare environments, including acute and elective areas, I’ve gained experience in communication and helping to plan intervention for a broad range of clients and service users. Throughout this time my professional focus has always been on “best patient experience ”. ‍My work has involved me frequently meeting clients and building therapeutic relationships, often over a short period of time. I am a Case Manager experienced in working with pre and post litigation cases.
I am also a Registered Physiotherapist with 30 years of clinical experience in a variety of health and social care settings. Having spent the first 6 years of my physiotherapy career working in areas such as neurology, medical and surgical, MSK, amputees, palliative care and women’s health, I moved into working in community rehabilitation.

This is where I developed my skills in working with a wide range of people from all backgrounds, in their own homes and communities, with conditions including complex care, falls, frailty, neurology, orthopaedics, supported hospital discharge and more. I am passionate about rehabilitation and experienced in helping clients and their families navigate complex health and social care systems to improve access to services.Collaborative multidisciplinary team working is one of the cornerstones of good rehabilitation and care and I am confident in both participating in and leading multidisciplinary teams to work together towards achieving clients goals.In my Case Manager role I have worked with clients living with a range of conditions including acquired brain injury, cerebral palsy, spinal cord injury, learning disability and sensory impairment.

Living with a long term condition can bring many challenges. By taking a thorough and empathetic approach to finding out what matters to each of my clients, I strive to improve their quality of life by finding solutions to challenges that work best for them and their loved ones. I particularly enjoy organising trips abroad for some of my clients and am adept at care planning and risk management.
In my spare time I enjoy spending time with my young son and being beaten at chess by him!

I am a Registered Nurse with a wealth of experience working in multi-disciplinary teams as well as autonomously.‍
During my career I have developed clinical skills and a knowledge base as a Registered Nurse, working in the community, care of the elderly, Gynaecology, and a GP surgery as a Practice Nurse. I further developed my career, gaining a PG Diploma in Specialist Community Public Health Nursing whilst working as a Community School Nurse, covering 15 schools both Primary and Secondary working with children, young people, their families, and other professionals to help improve the health and wellbeing of school aged children in the local community.‍
This included writing substantive reports and representation for early intervention, safeguarding and care planning.‍ I also work as an on-call Forensic Nurse Examiner in a Sexual Assault Referral Centre which involves working autonomously, providing an examination, taking medical and forensic samples, meticulous, accurate note taking and reports for the Police, legal representation and attending courts to be cross examined if required. ‍I am able to adjust my communication style to meet the needs of differing individuals and situations to ensure full participation of those individuals or groups that I am working with at any one time. ‍
This has been extremely important in previous positions where I have had to interact with individuals from across different social economic, ethnic/racial, and cultural backgrounds with different religions and beliefs.‍ Also, people of different sexual orientation/gender within the community, GP Practice, Schools, Hospital wards and in the Sexual Assault Referral Centre, all positions which require excellent interpersonal skills and require a non-judgmental, empathetic approach, something that I excel in.‍

I am a qualified and registered social worker who has a background of working with a wide range of clients, however, specialise in supporting children and young people.I have worked 8 years as a social worker and before this I worked in the care sector supporting disabled children and adults.
My passion is to provide holistic care with the aim to empower clients to achieve their goals and aspirations.
